Please read carefully before completing the 26/27 Group Election form online.

Because there is no option to save the form and return to it later, make sure you are prepared with the following information:

  • Tax ID
  • Elected official resolution (if applicable; click for a sample resolution for city or town)
  • Elections for the following coverages: medical, dental, vision, basic & voluntary life
  • Current number of eligible participants (as of today)
  • Method to determine eligibility (lookback or month-to-month) for employers with over 50 FTE employees
  • Current number of participants enrolled in the plan
  • Hours per week required to be eligible for benefits
  • New - The city/town's contribution to employee's benefits must be reported as a dollar amount instead of percentage

Group election forms should be completed online this year by Thursday, April 16, 2026. Once your form has been properly submitted, you will receive a confirmation email that will summarize your group elections.
